Documents that SEE YOU AGAIN prepares for you: the travel plan and tour itinerary, which also serves as your car rental booking (each person must print a separate set) Hotel reservations for every hotel (each person must print a separate set). Documents you must prepare yourself. Book your visa appointment yourself: you should book your visa appointment as early as possible, especially during festival periods, when slots are very limited. / You can apply for a Czech visa in 2 ways: 1. Book an appointment on the Czech Embassy website and submit your application at the Czech Embassy. 2. Book an appointment and submit your application through the visa application centre VFS CZECH REPUBLIC. Travel insurance: buy an individual worldwide policy that covers the period from the day you fly out of Thailand until the day you fly back to Thailand. Choose a policy with overseas medical coverage of 2 million baht. Travel insurance can be arranged through Khun Pukky / AXA Insurance Thailand Direct & Alternativ 081 926 4010 [email protected]. Visa fee: if you apply directly at the Czech Embassy, the fee is 60 euros per person, but it must be paid in Thai baht, so prepare the exact amount at the exchange rate on the day of application. / If you apply through the VFS visa application centre, you must pay the Czech Embassy visa fee of 60 euros plus the VFS service fee of approximately 600 THB, for a total of approximately 3,000 THB, paid in Thai baht and in cash only (credit cards are not accepted). English flight reservation: you do not need to buy tickets yet; a reservation is enough. Once your visa is approved, you must buy the flight tickets and show them to the embassy on the day you collect your visa. Visa application form: download it from the relevant embassy's website. Make sure it is the latest version, and fill it in neatly in English capital letters. For children, an adult may fill in the form, and the parents may sign on their behalf. Photos: 2 photos sized 3.5 cm x 4.5 cm (no glasses) with a white background only. Attach the first photo to the visa application form and clip the second one to the form. On the back of the photo, write your first name, surname and passport number (in English). Passport: the original current passport, valid for at least 6 months (counting from the planned date of departure from the destination country or the last day of your flight back to Thailand). If you have old passports, bring them along. Also make 2 photocopies of the current passport's photo page and sign them as true copies. If you have had a Schengen visa within the past 3 years, photocopy that visa page and sign it as a true copy as well. Also submit: your original ID card and a copy, signed as a true copy. If a young child cannot yet sign their own name, both the father and mother must sign the copy as true on the child's behalf. / For children under 6, use the birth certificate or ID card. On the day of submission, children do not need to attend; parents can fill in and sign the form and apply on their behalf (ID card copies do not need to be translated). House registration copy, signed as a true copy. If a young child cannot yet sign their own name, both the father and mother must sign the copy as true on the child's behalf (for children under 20, the house registration must be translated; for adults over 20, it does not need to be translated). Birth certificate copy for children under 20, signed as a true copy. If a young child cannot yet sign their own name, both the father and mother must sign the copy as true on the child's behalf. Because the father or mother will act as sponsor, the birth certificate must be translated as proof of the relationship with the parents (on the day of submission, bring the original birth certificate). Documents must be translated into English. Copies of marriage certificate, divorce certificate, name change certificate, surname change certificate, and birth certificate for children under 20 (if applicable). Documents must be translated into English. Financial evidence: use only a personal savings account. Company accounts, fixed deposit accounts and current accounts cannot be used. You need both of the following: an English bank certificate showing the account balance, issued by the bank no more than 7 days before the visa application date; and an English bank statement covering the past 6 months, issued by the bank (the account holder's name must be in English), issued no more than 7 days before the visa application date. The funds must be sufficient for the trip, for example 50,000 - 300,000 THB per person, depending on the length of the trip. (On the day of the visa application, bring the original bank passbook with the latest balance updated, in case the officer asks to see it.) If someone is sponsoring the travel costs, such as family / children / spouse: the main account holder who is the sponsor must personally type a sponsor letter in English stating that they will sponsor the trip and specifying the relationship, and attach copies of the sponsor's 2 financial documents, signed as true copies (the sponsor does not need to come to the embassy). Do not forget to attach a copy of the passport photo page of the person being sponsored (signed as a true copy), together with a document showing the relationship, such as a marriage certificate or house registration. - - If a couple is married but not registered, submit the children's birth certificates or write a letter of explanation in English signed by both of you. Employment certificate: an original letter in English stating your first name, surname, date of birth, position, start date of employment and leave dates (to show that you will return to work as before), with the company stamp. The translation does not need to be certified at a consulate. Business owners: attach the business registration certificate or company affidavit (original company affidavit only, using only the first page that shows the names of the company directors, issued by the Department of Business Development no more than 1 month before the visa application date; documents must be translated into English). Retirees: show your government pension card with a copy, signed as a true copy. Government officials: show your government official ID card with a copy, signed as a true copy. Students: show an original English student status certificate. Self-employed: write a letter of explanation in English describing the source of your income, and attach additional evidence such as a shareholder register or photos of your shop. Homemakers with no income: use copies of your husband's financial evidence instead of your own, together with an English sponsor letter from your husband stating that he will cover your expenses, a copy of his passport photo page, and evidence of the relationship, such as a copy of the marriage certificate (documents must be translated into English). If you are not legally married, submit your child's birth certificate or house registration instead, since it shows the names of both parents (documents must be translated into English). / Sponsors, please do not forget to sign every copy as a true copy. How to collect your passport on someone else's behalf after the visa is issued: A power of attorney if you are asking someone else to collect it for you / written in English. Copies of the ID cards of both the person granting authority and the person receiving it, signed as true copies. / On the collection day, the officer may ask to see the receiver's original ID card, so please bring it with you. The embassy receipt you received when you submitted the visa application, as proof for collecting your passport with the visa. Everyone must prepare a separate set; sets must not be combined.
